Fosfomycin trometamol is an antibiotic that kills bacteria by stopping them from building their cell walls, which causes them to die. It is unique because it usually works with just a single dose.
Inhibits bacterial cell wall synthesis by binding to UDP-N-acetylglucosamine enolpyruvyl transferase (MurA), preventing peptidoglycan precursor formation and causing bacterial cell lysis. Exhibits bactericidal activity against gram-positive and gram-negative uropathogens with rapid urinary penetration.

For adults, dissolve one sachet (3g) of the granules in half a glass of water and drink it on an empty stomach — ideally 2 to 3 hours before or after eating. You only need to take it once. It is best to take it at bedtime after you have emptied your bladder. One single dose is the full treatment for most UTIs. Sold in Nigeria as Monuril Granules.
Since fosfomycin is a single-dose treatment, there is usually no missed dose to worry about. If you forgot to take it or are unsure whether you took it properly, simply take your one dose as soon as you remember, preferably on an empty stomach.
Important for antibiotics
Always complete the full course even if you feel better. Stopping early can cause the infection to return and become drug-resistant.
Buy only from licensed pharmacies — avoid buying antibiotics from roadside sellers or open markets, as fake or expired drugs are common. Even though fosfomycin is a single-dose antibiotic, do not share it with someone else who has similar symptoms — they need to see a doctor for proper diagnosis. If your symptoms do not improve within 2 to 3 days after taking the dose, go back to your doctor as you may need a different treatment.
Metoclopramide (commonly used for nausea and vomiting in Nigeria) can reduce how well fosfomycin works, so avoid taking them together. Taking fosfomycin with food significantly reduces its effectiveness — always take it on an empty stomach. There are no major alcohol interactions, but it is wise to avoid alcohol while treating any infection so your body can recover properly.
